Medical marijuana is now legal in Utah for certain qualifying medical conditions. We are here to help you navigate the process of obtaining your Utah Medical Card. We can’t wait to help you feel better.
Utah Qualifying Medical Conditions:
Chronic Pain (persists longer than 2 weeks)
Acute Pain (expected to last for 2 weeks or longer for an acute condition)
Post-Traumatic Stress Disorder (PTSD) - requires documented diagnosis and regular follow up with your provider
Persistent Nausea
Cancer
HIV/AIDS
Alzheimer’s Disease
Amyotrophic Lateral Sclerosis (ALS)
Cachexia
Inflammatory Bowel Disease (Crohn’s Disease or Ulcerative Colitis)
Epilepsy or debilitating seizures
Multiple Sclerosis
Persistent and debilitating muscle spasms
Autism
A terminal illness when the patient’s life expectancy is less than six months
A condition resulting in the individual receiving hospice care
A rare condition or disease with limited treatment options

***If you are unable to leave your home or are on hospice care, please contact us for home visit arrangements.***
***If you are unable to walk up a flight of stairs, please contact us before making your appointment for other arrangements.***
If you are over 21 years old and it is determined that you do not qualify for a medical card, your full deposit/payment will be refunded.
If you are under 21 years old and require submission to the Compassionate Use Board, your deposit/payment is not refundable even if you do not qualify for a medical card.

Niki Davis, MD
Dr. Niki Davis is a board certified Family and Lifestyle Medicine physician as well as a licensed QMP (Qualified Medical Provider) in Utah. She graduated from the University of Utah School of Medicine in 2017 and completed her Family Medicine residency in Billings, Montana in 2020. She was born and raised in Salt Lake City and loves living here with her family. She is passionate about helping people live their healthiest lives.
